Standards of Practice: Printable versions of the forms that
accompany the standards of practice in PDF format.
Tax Rates: Forms that are necessary for a taxing entity to
obtain approval of tax rates are here. There are forms regarding taxable values,
taxes charged, taxes collected, taxes disbursed, redevelopment agency funds, new
growth, annexations, proposed tax rates, final tax rates and proposed and final
budgets.
Centrally Assessed - All Instructions and Forms: Each centrally
assessed taxpayer must send a completed "Annual Return for Assessessment"
to the Property Tax Division of the Utah State Tax Commission. For further
instructions and forms, click on the "Centrally Assessed - All Instructions and Forms" label to
the left.
Natural Resources Annual Reports (Returns): Each centrally
assessed Natural Resource taxpayer must send a completed "Annual Return for Assessessment"
to the Property Tax Division of the Utah State Tax Commission. For further
instructions, click on the "Natural Resources Annual Reports (Returns)" label to
the left.
Utility and Transportation Annual Reports (Returns): Each centrally
assessed taxpayer must send a completed "Annual Report and Annual Return for Assessment"
to the Property Tax Division of the Utah State Tax Commission. For further
instructions, click on the "Utility and Transportation Annual Reports (Returns)"
label to the left.

Most of our instructions and forms are in the Adobe Acrobat Portable Data File (PDF) format.

Note: the program is quite large and may take up to one hour
to download (if you're using a slow modem). If you have a fast internet connection,
however, it should only take 10 minutes or less. The Tax Commission also has a page
